Summary

The project aims to improve pediatric palliative care (PPC) by developing a telemedicine model centered on home-based care, promoting decentralization and accessibility. Multidisciplinary co-creation sessions, including different stakeholders will be used to identify needs assessments, facilitators and barriers, workflow analyses, and model refinement. A pilot study will evaluate the model’s effectiveness, adaptability, and impact on patient outcomes, caregiver satisfaction, and hospital dependency. The project will deliver evidence-based strategies for national and international scalability, supporting the transformation of PPC delivery systems.
